Each city/county has its own product approval specification sheet, but they are all essentially uniform with four columns to complete and begin with a brief introductory message to the tune of:

We recommend you contact your local product supplier should you not know the product approval number for any of the applicable listed products. Statewide approved products are listed online at www.floridabuilding.org.
